BIOLOGICAL STAFF ACCOMMODATION FORM

Members of staff are advised to visit University of Lagos Website to complete the staff biological form for the 2015/2016 accommodation exercise.



The form is to be completed and submitted online.  Thereafter members of staff are expected to print a copy for the Head of Department’s endorsement and later submit at the Student Affairs Office Division.



Members of staff must ensure that they keep a copy of the form submitted.



Closing date for the submission is Friday, January 8, 2016.


GENERAL GUIDELINES ON ACCOMMODATION OF HALLS OF RESIDENCE

The 2015/2016 accommodation exercise is strictly an online distribution and allocation of bed spaces.
2.      Students should note that there are limited bed spaces due to increase in the number of students.

3.      Allocation of bed spaces will be on the basis of  first come first served

4.      All freshers shall be distributed into permanent HoR. Thereafter they will ballot for bed spaces on the basis of their assigned HoR.

5.      Students with medical challenges (physically challenged inclusive) should register at the Medical Centre; otherwise their cases will not be treated by the Students Affairs Division.

6.      No sales of scratch cards for accommodation. Students are advised to log on directly to www.unilaghalls.com for allocation of bed spaces using matriculation number or jamb registration number as the case is applicable

7.      Students should strictly abide by the time table for allocation of bed spaces for each category.

8.      Illegal sales or purchase of bed spaces attracts rustication for two (2) semesters; any student caught will be disciplined accordingly, so beware!!!

9.      Students must pay their accommodation fee within 3-working days after allocation of bed space, else the bed space stands forfeited

10.  Deriving from nine (9) above, another round of balloting will be carried out to give opportunity to students who did not succeed in the earlier ballot.

11.  Complaints should be channeled through the online help desk for accommodation.  All complaints shall be attended to promptly. No students and staff are allowed to gather in or outside the Students Affairs Office.

12.  Re-assignment of HoR based on choice will not be entertained; students with such request shall forfeit his/her allocation.

13.  Under no circumstances should any bed space/room not officially assigned be occupied by any student.

14.  Students penalised for any form of social and academic misconduct are not eligible for accommodation.

15.  Biological form for staff child(ren) has been uploaded online, members of staff are expected to fill, print, sign and submit at the Student Affairs Division (attach copy of birth certificate and last course registration form of 2015/2016 session). Other necessary information will be posted on the University Website and in the Information Flash.

16.  All students resident in the HoR shall be issued identification pass/cards, which must be produced on demand at any point in time.

17.  All students are requested to avail themselves of information on accommodation through the Unilag Website, Lagmobile and the Information Flash.

18.  The date for balloting of bed space for freshers is Wednesday, December 23, 2015 while that of final year and other levels will commence in January.

As due to some things really happening due to the postponement of hostel allocation and balloting..
University Of Lagos has decided to cancel all lectures scheduled for this year for freshers and has refused to give out lecture time-table due to consideration of those leaving in far distances..

NEW SECURITY MEASURES ON CAMPUS


As we begin the Xmas period, Management of the University has directed that the following important security measures be implemented with respect to safety of lives and properties on campus.



1. a.) All staff and students are hereby directed to always display the university identification card on themselves while on campus.



b.)Visitors to some locations on campus would be required to wear visitors tag.



c.) The security personnel have been mandated to request for identity cards from all persons on campus.





Members of the University community are reminded of the following:



2. Report any unattended baggage to the nearest security personnel or call the following numbers 1048, 1767, 1081, 08023363711; University Fire Service at 08028434726 or 1088.





3. Be vigilant and report any suspicious movement to the above numbers.





4. Members of the community who intend to visit or work in the Senate Building/Main Auditorium or Multipurpose Halls are to note the following:



· Security screening is mandatory for everybody;



· All items including phones, bags and food (processed or otherwise) must be subjected to security screening;



· No entry shall be granted to anybody with any unscreened item;



·The screening equipments are among the best in the world, safe for both food and pharmaceuticals and have NO ADVERSE EFFECT.

Professor Folasade Bolanle Adepeju Akinsola of the Department of Ophthalmology, College of Medicine, University of Lagos will deliver the third (3rd) inaugural lecture in the 2015/2016 academic session on Wednesday, December 23, 2015 at 4.00p.m in the Main Auditorium, Unilag, Akoka



The Vice Chancellor, Professor Rahamon A. Bello, FAEng will preside over the lecture entitled: “Ophthalmic Care: Wherein a Public Health Ophthalmologist & Medical Trainer?”.  Members of the University community and the general public are cordially invited.

TIMETABLE

The allocation has been scheduled to hold on different date depending on the student category. It is therefore mandatory for every student to adhere strictly to the instructions guiding the hostel allocation. MIGRATION TO NEW RESULTS PROCESSING PLATFORM

This is to inform all students that the University's Semester  Examinations Results platform is now in a migration process. In order to ensure that ALL outstanding issues are addressed, COMPLETE semester examinations results have been regenerated for ALL RUNNING students.

All students who have not yet done so are advised to log in to their portals to verify that the records reflect their actual results. Where a student is satisfied that there is a mistake OF ANY VARIETY, he or she should complete the online complaints form, and patiently await the resolution, which will come about at the end of the 'view/complaint' period on FRIDAY, DECEMBER 18, 2015.

PRE-QUALIFICATION REQUIREMENTS

1A        Compulsory and Mandatory Requirements

i.              Evidence of company’s Certificate of Incorporation with the Corporate Affairs Commission (CAC).

ii.            Current Company Audited Account for the last three years (2012, 2013 and 2014) duly stamped by a professional accounting firm/Audit firm showing annual turnover among other financial information.

iii.           Evidence of Tax Clearance Certificate with minimum annual turnover of N10 Million for project No. 1 and N4 Million for project No. 2 for the last three (3) years (2012, 2013 and 2014).

iv.           Current Pension Clearance Certificate from National Pension Commission (PENCOM).

v.            Evidence of compliance with the statutory provision of section 6 (1)-(3) of the (ITF) Industrial Training Fund Amendment Act, 2011 by inclusion of compliance certificate from ITF.

vi.           A sworn affidavit indicating that no current staff of the University of Lagos or Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the company submitting bid, the company is not bankrupt and also confirm that all information presented in the bid are true and correct in all particulars.

vii.          Corporate profile to include list, curriculum vitae (CVs) and copies of credentials of Technical Staff with academic/ professional qualification with:

For Project No. 1:


1.    COREN, ARCON, QSRBN, CORBON including years of cognate experience of personnel available for the project (copies of Certificates to be attached) for the building project 1. The following are the minimum:

(a ) One (1) registered structural  engineer (with current practicing licence) of not less than five (5)  years registration with COREN OR

(b) One (1) registered builder (with current practicing licence) of not less than five (5) years post    registration with CORBON.

For Project No. 2:

1.    Relevant affiliation with medical equipment manufacturers and at least two (2) qualified Medical equipment technicians.

viii.        Evidence of payment of the Non-refundable pre-qualification fees of N20,000 at the UNILAG Cash  Office.



ix (a) Verifiable evidence of at least construction of 1 nos, bungalow building on raft foundation

         successfully executed or being executed in Nigeria over the last five (5) years stating type,

          cost with evidence of award and completion certificate for Project No. 1, and

(b) Verifiable evidence of similar supply and installation of modern medical equipment successfully executed or on-going in Nigeria in the last five (5) years stating type, cost with evidence of award and completion certificate for Project No. 2.

Please note that failure to provide any of 1A (i) – (ix) above will automatically disqualify the Bidder.

1B.       Other Requirements:

i.              Verifiable evidence of ownership or lease agreement of essential equipment in full working condition to be used for the project as applicable.

ii.             Satisfactory Bank reference from a reputable Bank demonstrating availability of fund and/ or credit facilities from the bankers to successfully complete the contract

The two (2) envelopes should be enclosed together in a third envelope and identified with the specific project contract number and title and submitted to;

F  The University reserves the right to verify the authenticity of any claim made in the pre-qualification document submitted by bidders.

F  The University Authority reserves the right to annul the selection process at anytime without incurring any liabilities and assigning any reason thereof.

F  Interested bidders that paid tender fee in previous advert, need not pay another fee, but must include a copy of the receipt as evidence of payment in their pre-qualification document.

F  The University will not be responsible for any cost incurred by the bidders in connection with the response to this advertisement.
